No character should ever be tranformed to "?" except "?". There are some tools which display characters they don't know as "?", but that isn't a Xalan issue.
If you've got a specific testcase, check the actual bytes in the file. If you can confirm that something is really being transformed into "?" that shouldn't be, submit a bug report.
